Transplant care represents one of health care’s most complex journeys. Patients must navigate a highly fragmented system — from initial evaluation and waitlist placement through transplantation and post-operative care — and success depends on adherence to complex treatment protocols.
This Optum white paper evaluates the clinical effectiveness and economic impact of case management among complex transplant recipients, comparing outcomes between actively engaged participants and non-engaged members. The analysis is based on a retrospective cohort study of commercially insured adults who received complex transplants between January 1, 2020, and December 31, 2024, and examines differences in cost, utilization and clinical outcomes.
